NR 451 Week 8 Collaboration Café: Reflect on Knowledge
The learning experience in this course and the change project offer vital lessons. Several concepts stood out and significantly impacted my understanding of the course and the role of baccalaureate-prepared nurses in enhancing access to healthcare, especially through nurse case management approaches on chronic conditions like diabetes and hypertension. The course concepts include nurse-led evidence-based interventions, health promotion and illness prevention, personal and professional development, and participation in professional organizations (Daff et al., 2024). Other concepts like holistic health principles, interdisciplinary knowledge and collaboration, and ethical decision-making are some of the concepts that I hope to apply in my future practice as a baccalaureate-prepared nurse. For instance, evidence-based approaches to nurse case management are essential in reducing the severity of chronic conditions among elderly individuals. I will use these concepts to improve care delivery and understand patient needs based on their situations.
The course program outcome 8 entails the incorporation of evidence-based practice to provide professional nursing care to individuals, families, aggregates, and communities. My learning in the course has contributed to attaining this outcome as it focuses on using telehealth as a case management intervention to improve health delivery for adults with chronic conditions like diabetes. The course has offered a strong foundation in holistic approaches that are essential for providing high-quality, safe, and patient-centered care, especially for individuals with chronic conditions. The course activities and assignments have provided an avenue to apply these principles in practical situations, aiding the development of core skills required for effective practice as a nurse and healthcare provider.
The American Association of Colleges of Nursing (AACN) Essentials (2024) enumerate competencies that nurses should attain through courses to be effective in care delivery. The course has assisted me to achieve competence 1.2 which requires nurses to use theory and research-based knowledge from nursing, the arts, humanities, and other sciences when delivering patient care. For instance, the integration of telehealth, knowledge about social movements, and the legislative process demonstrate the importance of using research and other sciences to enhance care delivery. The development of the change project demonstrates the practical application of course outcomes on evidence-based practice (EBP) and its importance in healthcare settings (Curley et al., 2024). The course has been fundamental in enhancing my understanding of these approaches and their application in practical circumstances.

Collaboration Café
Purpose
The purpose of this Collaboration Café is to reflect on course outcomes, program outcomes, and the American Association of Colleges of Nursing (AACN) Essentials to expand self-awareness, identify knowledge gaps, and assess learning goals.
Course Outcomes
This Collaboration Café enables the student to meet the following course outcomes:

- CO 3: Demonstrates effective communication with patients, colleagues, and other healthcare providers in managing safe/quality healthcare processes using evidence-based practices. (PO 3)
- CO 4: Integrates clinical nursing judgment using effective communication strategies with patients, colleagues, and other healthcare providers. (PO 4)
- CO 5: Develops a plan for continued personal and professional development through enrollment in graduate education, continuing education programs, certification attainment, professional reading and participation in professional organizations and service to the community. (PO 5)
- CO 6: Plans clinical practice activities that integrate professional nursing standards in accordance with the Nursing Code of Ethics and the American Nurses Association (ANA) standards of practice. (PO 6)
