Change projects aim at improving quality care outcomes for patients. In this case, the overall goal of the change project on using nursing care management by integrating telehealth is to enhance the quality of life for individuals battling chronic conditions like diabetes and hypertension. Inter-professional collaboration through a committee is an essential part of attaining the overall goal of this change project (Cardiff et al., 2024). As such, the inter-professional committee will include nurses, physicians, information technology experts, pharmacists, and social workers. The nurse will offer insights into the overall case management using telehealth to deliver care to patients in remote locations. The physician’s role would be to provide clinical perspectives and ensure that the intervention aligns with medical practices as mandated by regulatory entities. The information technology (IT) expert will develop the telehealth system and software in collaboration with external vendors while ensuring input for customization to meet the facility’s needs. The pharmacist will provide input on medication management and possible drug interactions while social workers will coordinate care with the stakeholders to attain set goals.
The primary role of the nurse would be to coordinate care and be at the center of care delivery. The nurse will provide patient-centered care interventions based on unique patient needs and requirements. The physician’s responsibility will be to evaluate and assess the impact of the new intervention in reducing the burden of chronic conditions on patients. The IT personnel will ensure that the developed software and system align with the organization’s electronic health records (EHR) system and meet patient needs through easy navigation. The pharmacist will evaluate the medication management process and its features to recommend any improvement (De Baetselier et al., 2021). The plan to inform the selected Council members about steps in the change process would include regular updates, either weekly or monthly, and presentations during formal and informal sessions. The information to the members will highlight the benefits of the intervention in enhancing the management of chronic conditions and helping the affected to freely consult with the team for an improved lifestyle.
Conflicts are an inevitable part of groups, including the Council and its members. However, developing mechanisms to resolve such situations is critical to the overall care delivery. Strategies to address conflicts in the council will include open communication, participative leadership, and active listening to encourage people to speak out and make recommendations. Again, having clear decision-making processes and engaging a neutral mediator or arbiter can help overcome these conflicts. Resistance to change occurs due to inadequate communication and motivation to embrace new approaches to care delivery (McLaney et al., 2022). In this case, strategies to address change resistance will include enhancing the engagement of all stakeholders, providing sufficient information about the benefits of the new system, and staff participation in the planning process.

Purpose
The purpose of this discussion is to explore the importance of interprofessional partnerships in the successful implementation of an evidence-based change project.
Course Outcomes
This discussion enables the student to meet the following course outcomes:
- CO 2: Proposes leadership and collaboration strategies for use with consumers and other healthcare providers in managing care and/or delegating responsibilities for health promotion, illness prevention, health restoration and maintenance, and rehabilitative activities. (PO 2)
- CO 3: Demonstrates effective communication with patients, colleagues, and other healthcare providers in managing safe/quality healthcare processes using evidence-based practices. (PO 3)

Paragraph one: Imagine you form an interprofessional committee to plan the implementation of your change project. Summarize the overall goal of your change project. Explain who is part of the interprofessional committee and why each team member is included. Choose only three Council member roles for this discussion.
Paragraph two: Describe each team member’s roles and responsibilities specific to the change project. Discuss your plan to inform Council members of the steps in the change process and how the change project would benefit from the expertise of these stakeholders identified in paragraph one.
Paragraph three: Explain how you would address conflicts among the interprofessional team members during the planning of the change project. Describe the strategies you could use to address resistance to change.
