Weekly Clinical Communication and Documentation Requirements
During the course of this week in clinical, I had the opportunity to interact with several patients who need both preventative and chronic care. As a result, my capacity to carry out complete assessments and devise appropriate management strategies was enhanced. An encounter that was particularly fascinating concerned a woman who was in the perimenopausal stage and was experiencing vasomotor symptoms. These symptoms included hot flashes and sleep difficulties. I gained an understanding of the significance of separating symptoms associated with menopause from those associated with other endocrine problems and taking into consideration treatment options that are both hormonal and non-hormonal. I also enhanced my documentation skills by making sure that my SOAP notes showed clear clinical reasoning and that there was congruence between the components of the plan, the subjective approach, and the objective approach. My confidence in my ability to manage issues related to women’s health was boosted by this encounter.
As I look to the future, I want to enhance my clinical decision-making skills, as well as boost the efficiency with which I interact with patients, while also ensuring that my paperwork is accurate. I intend to concentrate on increasing my understanding of evidence-based guidelines, particularly about the management of chronic illnesses such as hypertension and problems connected to menopause. The clinical schedule that I have proposed for the following week consists of three days that are approximately twenty-four hours long. A further objective of mine is to play a more active part in patient education and counseling, to ensure that patients have a complete understanding of their diagnosis and treatment regimens. In the future, when I am in clinical practice, strengthening these abilities will allow me to deliver care that is complete and more focused on the patient.
