Surgical site infections (SSIs) in healthcare are the selected nursing practice problem. SSIs are infections that affect patients who have undergone a surgical procedure. The infections affect the surgical incision sites or the tissues surrounding the incision sites. SSIs have considerable impacts on nursing and healthcare. This can be seen in increased care costs, prolonged hospitalization, morbidity, and mortality among the affected populations. SSIs are preventable in nursing practice. Nurses can implement evidence-based strategies such as bundled interventions to prevent and reduce the risk and rate of SSIs. The proposed project investigates the impact of bundled interventions on SSI measures such as length of hospital stay, hospitalization costs, and mortality. Therefore, this paper examines a qualitative and a quantitative study on the practice problem and the proposed intervention.

Dukes, K. C., Reisinger, H. S., Schweizer, M., Ward, M. A., Chapin, L., Ryken, T. C., Perl, T. M., & Herwaldt, L. A. (2024). Examining barriers to implementing a surgical-site infection bundle. Infection Control & Hospital Epidemiology, 45(1), 13–20. https://doi.org/10.1017/ice.2023.114

Methodology

This study adopted a qualitative study design. It was a qualitative evaluation that examined the contextual factors that influence bundle adoption and implementation. The authors used purposive sampling to select healthcare personnel who implemented an SSI bundle. In-depth, semi-structured interviews were conducted with 30 healthcare providers. The study was conducted in 2 academic, tertiary care hospitals, 3 academic-affiliated, community hospitals, and 1 unaffiliated community hospital in the United States.

Goals

The authors of this study examined the contextual factors that influence bundle adoption and implementation in the prevention and reduction of SSIs. The authors believed that understanding the factors would inform the successful implementation of SSI prevention bundles in hospital settings.

Data Collection

A medical anthropologist conducted in-person or phone semi-structured interviews with the study participants. The data was recorded and transcribed. Thematic analysis was used for data analysis. The interviewer took detailed notes during an interview with a participant who declined to be recorded. The transcripts were imported into MAXQDA10, a software used for qualitative data analysis and management.

Zarain-Obrador, L., Alonso-García, M., Gil-Yonte, P., Hijas-Gómez, A. I., Rodríguez-Villar, D., Martínez-Amores, B., Gil-de-Miguel, Á., Ruiz-Tovar, J., Rodríguez-Caravaca, G., & Durán-Poveda, M. (2021). Effect of a Surgical Care Bundle on the Incidence of Surgical Site Infection in Colorectal Surgery: A Quasi-Experimental Intervention. Revista de Investigación Clínica, 73(4). https://doi.org/10.24875/RIC.21000067

Methodology

The authors conducted a quasi-experimental study with a before and after analysis. The researchers used a population of patients who had undergone colorectal surgery at the Alcorcon Foundation University Teaching Hospital. The patients received a low-residue diet before surgery, prophylaxis with neomycin 1 g, and oral cathartic. The patients were divided into two periods, 2007-2011 for the pre-intervention and 2012-2017 for the post-intervention period. Descriptive analysis was used for quantitative variables. Qualitative variables were described with their frequency distribution and compared with Pearson’s Chi-square test or with Fisher’s exact test if they did not meet the developed application criteria. The study variables included sociodemographic factors, disease status of the participants, hospital stay, urgency and duration of the surgery, type of surgery, surgical contamination, and administration of antibiotic prophylaxis.

Goals

This study aimed to evaluate the effect of a surgical care bundle on the incidence of SSIs in colorectal surgery.

Data Collection

The researchers collected data on variables that included sex, age, comorbidities, hospital stay, interventions, urgency and duration of surgery type of surgery, surgical contamination antibiotic prophylaxis, and pre-operative preparation-related aspects and compared them with the presence or absence of infection and microorganism involved.

The purpose of this assignment is to conduct a comparison on different research designs to better understand their design and application. Understanding the different types of research design is important so that nurses can effectively apply evidence-based research into practice to address issues and offer better patient care.

You will utilize your approved nursing practice problem to complete the evidence-based practice project proposal assignments for this course and NUR-590, during which you will synthesize all of the sections into a final written paper detailing your evidence-based practice project proposal.

Review feedback from your instructor on the Evidence-Based Practice Project Proposal: Identification of Nursing Practice Problem assignment submitted in Topic 1. If your original proposed nursing problem was outside the scope of nursing practice or not conducive to an evidence-based practice project proposal, work with your instructor to identify a new topic prior to beginning this assignment. If your proposed topic requires revision, complete this prior to beginning this assignment.

Conduct a literature search on your approved nursing practice problem. Select one qualitative translational research article and one quantitative translational research article for this assignment. Using the “Translational Research Graphic Organizer,” present your proposed topic and complete the tables.
